{"id":50,"date":"2015-07-16T17:02:15","date_gmt":"2015-07-16T17:02:15","guid":{"rendered":"http:\/\/mccdemosite1.devsiteurl.com\/health-safety\/?page_id=50"},"modified":"2015-07-16T17:16:31","modified_gmt":"2015-07-16T17:16:31","slug":"immunizations","status":"publish","type":"page","link":"https:\/\/mccdemosite1.devsiteurl.com\/health-safety\/immunizations\/","title":{"rendered":"Immunization Requirements"},"content":{"rendered":"<p>In accordance with New Jersey law, all students carrying 12 or more credits who are 30 years of age or younger are required to submit a record of immunization against measles, mumps, rubella, and hepatitis B.\u00a0 These records should be submitted to the Health and Safety Office at Mill Gate, located in Lot 2.\u00a0 Failure to comply with this requirement will prevent you from registering for your next semester.<\/p>\n<p>Call the health office at 732.906.2530 for more information.<\/p>\n<p><strong>Required immunizations are:<\/strong><\/p>\n<ul>\n<li>Measles (2): The first dose must have been given after 1968 and after your first birthday (up to 4 days before is allowed).\u00a0 The second dose must have been given at least 30 days after the first dose<\/li>\n<li>Mumps (1): Given after your first birthday (up to 4 days before is allowed).<\/li>\n<li>Rubella (1): Given after your first birthday (up to 4 days before is allowed).<\/li>\n<\/ul>\n<p>The vaccine may have been given for each disease separately, or it may have been given in combination form as an MMR injection.\u00a0 MMR injections given prior to your first birthday do not meet the requirement.<\/p>\n<ul>\n<li>Hepatitis B: A series of two or three shots given over a six-month period.\u00a0 The number of shots required depends on your age when they were received.\u00a0 Your healthcare provider can give you more details.<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><strong>Acceptable proof of immunization can be:<\/strong><\/p>\n<ul>\n<li>The Middlesex County College immunization form<\/li>\n<li>If you cannot provide the Middlesex County College immunization form, please submit one of the following:\n<ul>\n<li>A record from your physician<\/li>\n<li>A school record<\/li>\n<li>A public health department document<\/li>\n<li>A military record<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>The proof must be a document that specifies month, day and year of immunizations or the results of blood tests (titer) showing immunity to each one of the diseases (measles, mumps, rubella, and hepatitis.\u00a0 The lab reports must be submitted for the titers. \u00a0Your name and birthdate must be on the record and include your student number on the form.<\/p>\n  <div class=\"green-btn-small green-btn-medium\"><a href=\"http:\/\/mccdemosite1.devsiteurl.com\/health-safety\/wp-content\/uploads\/sites\/44\/2015\/07\/MCCImmunizationForm.pdf\" ><span>MCC Immunization Form<\/span><\/a><\/div>\n<p>&nbsp;<\/p>\n<p><strong>Two exemptions<\/strong> are allowed and must be certified as prescribed by law.\u00a0 They are:<\/p>\n<ul>\n<li>Medical reasons, such as immunosuppression issues, pregnancy or allergic reaction to neomycin;
